Radiofrequency has been used in medical aesthetics for some time and is a recognised technology for the treatment of fine lines and wrinkles and lax and sagging skin on a wide variety of body areas. During a radiofrequency (RF) procedure the human body conducts the current of the RF energy, the heat generated induces an immediate contraction of the collagen which in turn leads to a tightening of the tissue.
Radiofrequency is also used for fractional skin resurfacing (FSR) treatments. Traditional fractional resurfacing technologies use light, in the form of lasers to heat and damage the skin. This causes a healing response which induces the formation of collagen and elastin.
Ablative fractional lasers disperse heat into the skin in narrow columns as illustrated below; the energy used is high so the heat generated through the course of the procedure also reaches high temperatures. This heat causes both epidermal and dermal ablation and can be painful for the patient due to the levels of heat generated. Ablative fractional lasers result in a considerable downtime for the patient and there is an increased risk of post inflammatory hyper pigmentation post procedure.
When looking at current radio frequency fractional resurfacing machines there are clear limitations to the available technologies. Bi-polar RF fractional resurfacing (illustrated below) uses the RF energy to ablate the epidermis immediately around the electrodes. The dermal heating achieved is fractional rather than uniform through the skin and due to the superficial flow of RF, the energy can be limited.
Limitations faced by current RF and laser FSR systems include high levels of pain for the patient as a result of the high temperatures used, a lack of uniformity in the dermal heating and ablation and a generally reduced safety profile, with an increased risk of post treatment infections and PIH due to the high levels of ablation needed to produce a significant result.

The study, a link to the complete paper can be found below, looked specifically at the treatment of atrophic acne scars and concluded that not only was this approach effective for this treatment indication but also that the dry microablation provided by fractional resurfacing using radiofrequency reduces downtime and minimises the risk of side effects and infection comparative to using laser resurfacing methods.
